Case Work Copy

  • Write your note using the QST Treatment SOAP Note template (link is in “Important Links” at the top of the Supervision course) 
  • Post Case Notes 5 & 6 to the Discussion Forum topic with your name immediately after each treatment session.
    Enter the Forum
  • Include techniques used, adjustments made, what you did, what the parents did, and how did the child respond. 
  • Notify your supervising Master Trainer that you posted your note. 
  • Review the feedback from your supervising Master Trainer before your next Tx. 
  • Schedule sessions 7 & 8 (You will videotape Tx #8 and share it with your supervising Master Trainer for feedback. Please coordinate the technology to be used for this task with them).
  • Mark this topic as complete after completing the requested tasks.
