Case Work Copy

  • Write your note using the QST Treatment SOAP Note template (link is in “Important Links” at the top of the Supervision course)
  • Post Case Notes 3 & 4 to the Discussion Forum topic with your name immediately after each treatment session.
    Enter the Forum
  • Include techniques used, adjustments made, what you did, what the parents did, and how did the child respond.
  • Notify your supervising Master Trainer that you posted your note.
  • Review the feedback from your supervising Master Trainer before your next Tx.
  • Schedule sessions 5 & 6.
  • Mark as “complete” after you posted both notes and reviewed the feedback.
